Your Home Health Speech Therapy Care Plan

Our licensed and experienced SLPs work with you to create an individualized plan to meet your specific goals. Depending on your needs and situation, your individualized plan may include:

  • Exercises to strengthen muscles
  • Non-verbal communication techniques
  • Recommendations for eating positions or dietary changes

Speech Therapy Home Visits

What To Expect During Your First Visit

During your first speech therapy home visit, your home health SLP will take a complete medical history. This includes asking questions about the recent events that prompted you to seek speech therapy care. They may perform a few simple exercises in expressive (speaking) and receptive (understanding) language to make their assessment.

Your SLP will take all of this information and create a personalized care plan specific to your needs. They’ll review this care plan together with you and/or your caregivers to ensure that everyone agrees with the treatment. Care plans typically include:

  • Areas that need addressed
  • Exercises to address your needs
  • Independent exercises you can practice between sessions
Subsequent Speech Therapy Home Service Visits

After the initial visit, your SLP will visit you in your home as needed. Between visits, you should complete speech therapy exercises that your SLP assigns, since they will help you achieve your goals more quickly.

The duration of your overall treatment depends on your ability to meet your goals and maximize your potential.
