Nursing Care in Home

Beth Israel Lahey Health At Home’s nurses and advanced practitioners help manage and treat a wide range of medical issues. We work with you, your doctors and your loved ones to design a care plan that meets your specific needs, all within the safety and comfort of your own home.

How Skilled Nursing Home Health Works

Home care nurses are experienced in treating a multitude of health conditions. We’ll teach you or your loved one about your condition, how to manage it and how to maintain optimal health. For example:

  • If you’ve undergone surgery, your home care nurse will help you understand how to watch for signs of post-surgical complications, including blood clots and infection. We also help manage associated pain and care for post-surgical wounds.
  • If you have an ostomy, your nurse can help you learn how to manage your ostomy. We offer specialists to help with complicated situations.
  • If you or your loved one starts a new medication, your home care nurse can help educate you about the medication to ensure it’s working as expected. We’ll help you understand what the medication is for, how it works, possible side effects and when to take the medication.
  • If you need to use an inhaler or nebulizer, your nurse will review how to properly use these devices.

Health Education

Home care nurses provide you and your family with relevant health education, such as:

  • Diet related to diabetes or heart disease.
  • Exercise plans for your health condition.
  • Smoking cessation.

Health Care in Home Specialized Care

We offer specialized clinical programs designed to care for ongoing health conditions, including:

  • Chronic disease management. Beth Israel Lahey Health At Home providers help manage chronic conditions, such as diabetes, through our specialized disease management services.
  • Chronic respiratory disease. Our dedicated Chronic Obstructive Pulmonary Disease (COPD) Program helps you manage symptoms and treatment of COPD and other pulmonary diagnoses.
  • Heart failure care. We can help you with the symptoms and self-management of chronic heart failure and other cardiac diagnoses, to promote wellness and prevent hospitalizations.
  • Medical monitoring services. We offer remote patient monitoring services to help you stay safe in your own home.
  • Medical social workers. We offer medical social worker services if you need help connecting with available community services. These individuals can provide emotional support and help navigate complicated family dynamics that may impact your health.
  • Wound, ostomy and continence care. The Beth Israel Lahey Health At Home team includes nurses who hold Wound, Ostomy and Continence Nursing Certification (CWOCN) and Wound Care Certifications (WCC) for our field nurses. They have the specialized skills, knowledge and experience to provide expert care.

Wound Care

We provide wound, ostomy and continence home health care, including colostomy care, stoma care and home health wound care.

In-Home Skilled Nursing Visits

What to Expect During Your First Visit

When your home health nurse arrives for your visit, they’ll start by taking your medical and medication history. This includes asking you about recent events that prompted you to seek in-home nursing care. Your nurse will assess your ability to care for yourself, including managing your medications and daily tasks, such as getting dressed and bathing. Your nurse also will assess your overall home situation, including family and caregiver support and your mobility within your home.

Using all of this information, your home health nurse will develop a customized care plan for you. They’ll explain the plan with you and your family to ensure it meets your needs and goals, and that you agree with the plan. Depending on your situation, the plan may include other in-home services to help keep you safe, such as physical therapy or occupational therapy.

Subsequent In-Home Care Services

Depending on your needs, your nurse will continue to visit your home until the goals set in your care plan are reached or you’ve reached your maximum potential.
