With Beth Israel Lahey Health At Home’s telemonitoring services, a clinician automatically and securely monitors your daily health data using remote technology. If we receive data that’s unusual or concerning, we’ll reach out to ensure you get appropriate care in a timely manner.
The goal of remote patient monitoring is to help keep you or your loved one safe and healthy in your home, as well as to avoid future hospital stays.

If you need remote patient monitoring services, you’ll be contacted to schedule an appointment for setup. During this appointment, the installer brings the equipment to your home and teaches you, your caregivers and your family how to use it.
You’ll receive an easy-to-use computer tablet that shares information with Beth Israel Lahey Health At Home using a cellular data connection. You don’t need to have wireless internet to use the service. The installer also will set up a blood pressure cuff, pulse/oximeter meter (to check your heart rate and oxygen level) and a scale. These devices are all Bluetooth-enabled and automatically connect to the tablet.
Your care team will request that you test each morning using the equipment provided. If they notice anything concerning, our telemonitoring team will call you to retest or provide instructions for follow-up. We connect directly with your medical providers to send trend data on your vitals, get follow-up orders and help make any appointments you may need. When necessary, we can send a home care nurse to your home for additional care.
If you have heart failure, you may be offered a videoconferencing nurse visit via your tablet each week. The goal of these virtual visits is to increase the amount of contact you have with your care team and identify any potential problems quickly.
