How To Refer for Patient Home Care
For your convenience, we offer multiple ways to refer a patient for home health care:
- Call our intake specialists at 781-756-2488.
- Fax the referral to our intake office at 781-756-2489.
- Use Beth Israel Lahey Health Epic to submit a referral, if you’re within our health system.
Follow Your Patient’s Care with Us
When you refer a patient to us, we’ll partner with you to provide them with the best home care possible. We offer several ways you can continue to monitor your patient while they’re in our care:
- If you’re within the Beth Israel Lahey Health system, you can follow your patient’s care through Epic.
- You can access your patients’ notes through a link within Epic. Non-Epic providers can access information through CareLink.
- During our home health visits, we regularly communicate with you over the phone.
- After we’ve discharged your patient from in-home care services, we provide you with a care summary.
